If your Sim had acne during their teenage years and you didn’t take care of it, and now they’re a young adult with persistent acne, you’re not alone.
In CAS, there’s no option to remove or conceal acne as an adult. There’s no magic button to get rid of it or hide it.

What to Do if Your Teen Sim Has Aged up With Acne
To get rid of acne on adult Sims, while in live mode, open Game Options, go to pack settings, scroll to High School Years, turn off the acne option, and apply the new settings.
No need to save or restart. Just go back to live mode.
This should remove acne from your young adult Sim’s face.
If this workaround doesn’t work, have your Sim freshen up or take a shower after turning off the acne option.
If the acne doesn’t budge, give this a shot – it should do the trick.
The “Apply Facial Cleanser” mirror interaction is no longer available once your teenage Sim becomes an adult.
Don’t worry, acne isn’t permanent on adult Sims even if it wasn’t addressed during their teenage years.
Just turn off acne in settings, go into Live mode, and freshen up your Sim to make it disappear.
